What documents do I need to bring to be seen?

What documents are required for a work-related injury patient?

  • Hard copy Authorization To Treat form (to be completed by employer and on site at clinic at the time of patient registration)
  • Employer Information form (to be completed by employer prior to patient registration)

What is the policy for minors?

A patient under the age of 18 must be accompanied by a parent or legal guardian. If the accompanying adult is not the parent or legal guardian, he must have a completed Authorization To Treat A Minor Form, signed by the parent or legal guardian authorizing treatment and billing for services rendered, as well as a copy of identification of the signing person. The parent, guardian, or adult will also need to provide photo identification, proof of insurance, and proof of guardianship if applicable.
